Who defines what safe is? And who defines what risk is? If it’s not up to us to define what “safe” is, and what “risk” is, then whose job is it to decide that for us? There is no such thing as safe. There is no such thing as zero risk. How you personally view risk depends on your own circumstances, your own boundaries, your own clinging onto any control that you think you have over your life… And when society describes one action as safe and one as risky, they are not leaving room for the subjective experience of the individual. Everything is uncertain. Nothing is secure. Life is a massive scary adventure, and even when something appears rock solid, like your health, your family, your marriage, your birth, it’s not. You are taking a chance on all of it. Yes, there are things you can do to improve your chances of success in a lot of those areas. A great example of something that makes a lot of people feel safe, without increasing true safety, is giving birth in the hospital. 1. Remain calm. Easier said than done, for sure... but staying level-headed is a great way to keep the ball in your court when looking for a new provider. 2. Send an email to your old provider canceling future appointments. Tell them that you are switching your care and would like them to email you your chart. (Email is a great way to avoid a confrontation that might put you in a negative emotional space.) 3. When trying to find a new provider, stay positive — as in, don't bash your old provider. Keep the conversation focused on why this new provider is such a great fit. 4. Go into your appointment with your new provider knowing what you want and don’t settle for less. It's amazing how other people can sense your level of confidence, and when you have self-respect and set the bar high, others typically follow suit. 5. Remember that a hospital will NEVER turn away a laboring woman. So, if you're really late in the game, don't ever fear that you'll be left alone to labor without options. It’s a time when you can either realize that there is a greater power within you that you can turn to for the rest of labor… or that the power within you is too much, and you start fearing that it’s something that could overtake you. In birth, this is typically the time when we say "I can't do this anymore. I need help. Give me something. Help me.” Or if you're giving birth at home you ask to go to the hospital. Measuring the fundal height is a great substitute for an ultrasound for those who want to avoid it. This is the measurement from the public bone to the top of the uterus, which is called the fundus. At 12 weeks, the fundus should be just above the pubic bone, and by 16 weeks it should be growing at a rate of about one centimeter per week. Starting at week 20, the fundal height in centimeters should correspond with the number of weeks gestation (+/- 2 centimeters). If your fundal height grows much faster, it could be an indicator of twins, and if it grows less it could indicate high blood pressure or an infection. If your care provider does these things, it may be time to find a new one…🚩🚩 Make you feel powerless - Your provider works for YOU. They should give you information and adhere to your decisions. If they say things like “I won’t let my patients…” they are assuming power over your birth experience. Routinely does vaginal exams late in pregnancy - Vaginal exams in late pregnancy mean absolutely nothing. You can have an unchanged cervix and go into labor that day OR you could be 3cm dilated for weeks before the baby chooses to start labor. They encourage 39 week inductions or draw a hard line on due dates. Insisting on an ultrasound in late pregnancy - Third trimester ultrasounds are linked to more interventions but do not result in significantly lower risk of adverse outcomes. The Bishop score is a system used to decide how likely it is that you will go into labor soon. They use it to determine how likely it is that an induction will result in a vaginal birth. The score considers different factors and each factor is given a grade, and then these grades are added up to give you an overall score. Understanding your score: There are several factors that your provider will consider when calculating your Bishop score: Dilation of the cervix. This means how much your cervix has opened in centimeters. Position of the cervix. As your baby descends into your pelvis, the cervix moves forward. Effacement of the cervix. This means how thin your cervix is. It is normally about 3 centimeters long. It gradually becomes thinner as labor progresses. Fetal station. This is how far up the birth canal your baby’s head is. Consistency of the cervix. Whether your cervix feels soft or firm. If your Bishop score is high, it means that there’s a better chance that an induction will lead to an established labor pattern. If your score is 8 or higher, it’s a good indication that spontaneous labor might start soon. If your score is between 6 and 7, then it’s unlikely that labor will be starting soon. An induction may or may not be successful. If your score is 5 or below, it means that labor is even less likely to spontaneously start soon and an induction is unlikely to be successful. I've been on here for months now and realized I've never really told my story or shared where my passion for physiological birth comes from - it comes from my own story, my lived experiences. This induction in 2017 was the most traumatic experience of my life so far. But, I didn't realize that until years later in 2021 after my redemptive home birth. Anyways, this induction was the first step in the cascade of interventions I experienced. From start to finish, my induction was 32 hours long. I was very close to getting a c-section in the end after all of that. Every intervention led to more interventions that could have all been avoided if I had done any research into how birth is supposed to work prior to going into labor. But, I trusted the medical system. I will go into this experience more in other posts, but I just wanted to let you into my world a bit. I also want to remind anyone reading this that although you may have had a traumatic birth already or have heard horror stories from others, you can have an amazing birth where you are in control. It just takes a little work and trust in the birth process. Your big-picture birth vision is what you need to focus on every step of the way. When you can get clear on why it matters to you, it’ll make it very clear what kind of work you’ll need to do leading up to your birth, whether it be mindset work, overcoming fears, or diving into and really understanding your options in birth. So many things to think about during pregnancy so these are a few things I choose not to worry about. Due dates are guesses. If you know the exact moment you conceived then ok use a due date, but no one really knows that. It's hard to tell if you're going to have a big baby from an ultrasound, they can be off by a pound or more sometimes especially later in pregnancy. There is no perfect amount of weight to gain, everyone is different. I would just focus on whole foods. When we can surrender to the flow of birth, life can come through us simply and gracefully if we allow it. This level of surrender will take your birth to a whole new level. One with no fears, and no freakouts. But though surrendering in birth is simple, it’s not going to be easy. To be clear, there are no benefits to routine episiotomy, and the risks include perineal injuries, pain (short and long-term), blood loss, infection, and incontinence. There are some medical indications for an episiotomy (though it should ALWAYS be YOUR call), and those include: Fetal distress Shoulder dystocia (there are other ways to fix this besides cutting an episiotomy) Forceps delivery Maternal exhaustion (this is totally your call, and will most likely only be shortening labor by a few contractions) You need to know that there are providers who perform episiotomies without consent. This is absolutely something that you should discuss with your care provider before your birth. Your takeaway? This is my placenta from my third birth. I had never seen one in real life before, even from my first two births in the hospital. Why is saying "no" SO damn hard?? Start to understand that “no” doesn’t have to be an inherently negative thing. Despite what your limbic brain might think, “no” doesn’t have to be to rejection. When used simply and with honesty, it’s a statement about your current situation. That’s it. Your provider might not be used to hearing “no”, but you can assure them that your “no” is about you, and not about them. Here are 2 practical tips to get you to a place of calm refusal: 1. Be prepared! Don’t rely on your refusal skills to be top-notch if you’ve never practiced. It might feel awkward at first, but that’s exactly why you should. 2. Keep it simple It’s tempting to offer a justification for your refusal. It's “polite” and it “takes the sting out of the rejection”, and it’s totally unnecessary. Be warned: offering a reason for your “no” changes the dynamic. I think there's some confusion about what a doula is...so let me try to explain. Basically, a doula is a NONCLINICAL support person that provides emotional support, advocacy for the mother/partner, and education during pregnancy, birth, and immediate postpartum period. Unlike most clinical providers, doulas are with their clients for the entirety of their labor. Doula support looks different for everybody, but some examples of how I can support you are: 🌿Help you navigate your pregnancy and approach conversations with your provider 🌿Help you cope with early labor and decide when it is time to go to your birthing place of choice 🌿Help you avoid unnecessary interventions 🌿Provide hands on support during and after labor 🌿Assist with lactation once the baby has been born 🌿Help you process your birth and adjust to becoming a parent I DO NOT have a medical degree, therefore I am NOT here to give you medical advice. Not only will your pelvis open, because it’s not one bone, but your baby’s head will also mold because their skull bones are not fused, so their skull can easily overlap and create the right shape to get through the pelvis. More often than not, it’s the pelvic floor muscles that have more to do with letting the baby come down and out.
